Family Medical Centers follows the Primary Care Medical Home (PCMH) model of providing healthcare to our community. This simply means that we are a team-based health care delivery model that is intended to provide comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes.

Even though you may prefer to see only one specific provider when you come in for your office visit, there is an entire team of doctors, nurse practitioners, nurses, and various other support staff working together to ensure that you have the best possible health outcomes.
